Vince Lombardi. Not only because I'm a Packers fan but because of how he changed the game from a strategic standpoint and a player / coach relationship standpoint.

This question seems unusually difficult to answer. It's impossible to separate a manager and success, but there are great teams w/ mediocre managers and fantastic managers who had terrible teams. And then there's experience bias - hard to judge a manager w/o actually seeing many of a team's actual games. So much of what makes a manager excellent is tactical.
I love Billy Beane and what he's doing w/ the A's. I'd call him the greatest modern era general manager in baseball. That's about as definitive as I can get.

John Wooden at UCLA. The structure of college basketball is such that the coach has almost complete power over the team, and Wooden's combination of coaching, recruiting and leading built a lasting institution while winning 88 straight games and 10 NCAA championships (7 in a row) in the process. He competed against the entire collegiate ranks, not just a 20-30 team professional league, and triumphed despite the inherent luck and craziness in the NCAA tournament.

I wanted to include "managers" too, I've never really understood there to be a difference between a manager (futbol, baseball) and a coach (everything else?).
